Patient Guide

Welcome to Mindcore MH. We know that starting mental health care can feel overwhelming, so we’ve put together this guide to help you know what to expect — from booking your first appointment to ongoing care.

Before Your First Appointment

1. Book Your Appointment Schedule online or call our office at (718) 307-5627. Let us know if this is your first visit so we can allow enough time for your intake.

2. Complete New Patient Paperwork Prior to your visit, you’ll receive intake forms (medical history, current medications, insurance information, and consent forms). Please complete these at least 24-48 hours before your appointment so your provider has time to review them.

3. Gather What You’ll Need

  • Photo ID
  • Insurance card (if applicable)
  • List of current medications and dosages
  • Names and contact information of any other treating providers
  • Payment method for copay or self-pay fees

4. Verify Your Insurance (If Applicable) Contact your insurance provider ahead of time to confirm coverage for psychiatric or mental health services, including copay and deductible amounts. Our team can also help verify benefits — contact us at general@mindcoremh.com or (718) 307-5627.

What to Expect at Your First Visit

Your first appointment (“intake”) is typically longer than follow-up visits and focuses on getting to know you. During this visit, your provider will typically:

  • Review your medical, psychiatric, and family history
  • Discuss current symptoms, concerns, and goals for treatment
  • Talk through previous treatments or medications, if any
  • Begin developing a personalized treatment plan together

This is also a good time to ask questions about our approach to care, what to expect from treatment, and how we’ll work together going forward.

Appointment Types

  • In-Person Visits — Contact us at general@mindcoremh.com or (718) 307-5627 for our office location and directions.
  • Telehealth Visits — Secure virtual appointments available for eligible patients. You’ll receive a link and instructions prior to your appointment.

Ongoing Care

  • Follow-Up Appointments: Frequency depends on your individual treatment plan and will be discussed with your provider.
  • Medication Management: If prescribed medication, your provider will monitor progress and adjust treatment as needed at follow-up visits.
  • Between Appointments: For non-urgent questions, you may contact our office at (718) 307-5627 or general@mindcoremh.com. Please allow 1-2 business days for a response.

Cancellations and Rescheduling

We understand that plans change. If you need to cancel or reschedule, please notify us at least 24 hours in advance by calling (718) 307-5627 or emailing general@mindcoremh.com. Late cancellations or missed appointments may be subject to a fee.

Billing and Insurance

  • We accept select insurance plans as well as self-pay. Contact us to confirm your specific coverage.
  • Copays are due at the time of service.

In Case of Emergency

This practice does not provide emergency or crisis services.

If you are experiencing a medical or psychiatric emergency, call 911 or go to your nearest emergency room.

If you are experiencing a mental health crisis, you can also call or text 988 to reach the Suicide & Crisis Lifeline, available 24/7.

Patient Rights and Privacy

Your privacy matters to us. Please review our Privacy Policy and HIPAA Notice of Privacy Practices (provided during intake) to learn how your information is protected and used.
